* Office visit charges for students who have not been assessed the Health Fee are based on complexity of visit. Lab and professional radiology services are billed separately by the provider.
** Physical exam charges are based on complexity of exam. Laboratory services, if applicable, are not included in the cost, and are billed separately by the provider. Immunizations and chest X-rays are also billed separately.
*** Students who are 17 or under and enrolled in Cardinal Care will not be charged for a PPD test (legislation under the Affordable Care Act)
