Medical Student and Physician Assistant Entrance Requirements
- Download the following Stanford Immunization Form for Medical and Physician Assistant Students:
Immunization Form (61.75 KB)
- Have the Immunization Form completed and signed by your healthcare provider
- Enter all the immunizations from the completed form into your electronic health record via the Vaden Patient Portal
- Upload the Immunization Form and/or laboratory evidence of immunity to your electronic health record via the Vaden Patient Portal
- The following immunizations are required for all Medical and Physician Assistant students
- Requirements and screenings are based on current Centers for Disease Control and Prevention (CDC) guidelines

Measles Mumps Rubella (MMR)
Stanford requires written documentation of one of the following:

2 doses of live vaccines MMR/MMRV or
-
Laboratory evidence of immunity1
Varicella
Stanford requires written documentation of one of the following:

2 doses of Varicella vaccine or
-
Laboratory evidence of immunity1
Hepatitis B
Stanford requires written documentation of one of the following:
-
Three doses of Hepatitis B vaccine
-
Laboratory evidence of immunity from vaccination. (Hepatitis Surface Antibody)

Laboratory evidence of natural immunity (have had past hepatitis B infection)1 This must include results for Hepatitis Surface Antibody, Hepatitis B Core Antibody and Hepatitis B Surface Antigen
1Equivocal antibody titers are not considered sufficient to protect from infection and a complete vaccine series will be administered as recommended by the CDC-ACIP.
Tetanus, diphtheria, pertussis (Tdap)
Stanford requires written documentation of the following:

Tdap: Booster must be within past 10 years, regardless of last Td vaccine
COVID-19 Vaccine
Stanford requires documentation that can be uploaded into your medical record via the Vaden Patient Portal. The deadline for uploading proof of vaccine is August 01, 2022.
If Stanford’s COVID-19 vaccination requirement is not met, and a request for extension or exemption is not received by the deadline (August 1, 2022), a registration hold will be placed on your Axess account in advance of Axess opening for fall class registration. The hold on Axess will prevent the ability to register for classes.
Fees
Students are responsible for the cost of all entrance requirements.
Recommended Vaccines
In addition to the required immunizations we also recommend that you consider the following immunizations if applicable:
- Hepatitis A vaccine.
- Human Papillomavirus vaccine.
- Meningococcal Vaccine.
- Pneumococcal Vaccine (if you have a history of asthma or other lung diseases, have immune issues, or are a smoker).
- A completed polio vaccine series.
