To grant release, or request a copy, of your Vaden Health Services medical and/or mental health records with dates of service prior to September 1, 2026, please use the Vaden Health Services Authorization for Release of Health Information form.
Once completed, the form can be returned to:

- By Postal Mail: Vaden Health Center, 866 Campus Drive, Stanford, CA 94305, Attention: Medical Records Department
- In person at Vaden during business hours
Please allow up to 15 days for completion of your request. Fees may apply.
For release of Vaden Health Services medical or mental health records with dates of service beginning September 1, 2026 and forward, please access your Stanford Health Care MyHealth account.
